Membership Information
Membership Eligibility☐ A primary diagnosis of a Serious or Persistent Mental Illness (SPMI)
☐ 18 years of age and older
☐ Demonstrates safe behavior in the community without supervision
☐ Independent in mobility and self-care without supervision
Membership Process
Applicants need to fill out the Lighthouse on Marshall Application and a Release of Information form for their county case manager as well as a Media Release/Denial form. The county case manager will fill out the Membership Support form, the SPMI Verification form, and the Activity Levels form. If an applicant doesn't have a county case manager, they can ask another provider, like a therapist or psychiatrist, to help with these forms.
